Immigration evaluations for U Visas are offered for a set fee of $1100 and include the following (this includes a thorough review of your records including any police or hospital/medical records):
* Combined Intake/Testing session with a detailed background history combined with a number of psychological instruments to assess your current symptoms 

 * Feedback session for discussion of your results and recommendations

 * Report sent to you within a week after the feedback​​

Immigration evaluations for the N-648 (Medical Certification for Disability Exception) are offered for $1100 ($600 for the evaluation and then $500 to complete the form if you meet criteria - please note that exception is not guaranteed and the N-648 form will not be completed if your don't qualify which is why I split the payment). The evaluation includes a thorough review of your records such as any prior psychological reports/ hospital/medical records):
*  Initial Intake session with a detailed background history/records review

* Testing session with a number of psychological instruments to assess cognitive/neuropsychological functioning 

 * Feedback session for discussion of your results and recommendations

 * Completed form sent to you within a week after the feedback (if criteria is met for exception)​

→ All sessions are telehealth only and conducted online via Zoom, which is a secure HIPAA compliant platform. Please ensure a reliable internet connection and a quiet, private area in which to connect (coffee shops are not appropriate). Headphones/earbuds are helpful if you wish to tune out outside noise.

→ Due to licensure rules, psychologists are only allowed to practice if they have permission in the state in which the client is located at time of contact. I can currently complete telehealth assessments for clients located in GA, NM, and all of the PSYPACT states (currently: AL, AZ, AR, CO, CNMI, CT, DE, DC, FL, GA, ID, IL, IN, KS, KY, ME, MD, MI, MN, MO, ND, NE, NV, NH, NJ, NM, NC, OH, OK, PA, RI, SC, SD, TN, TX, UT, VA, VT, WA, WV, WI & WY but please see https://psypact.org/page/psypactmap for updated information as new states are being added monthly).
